Our network of 45 physicians in the Bathurst North Family Health Organization is spread out across Vaughan, Richmond Hill, Thornhill, and North York. We have coordinated our schedules to give you the highest quality of care possible after hours and 7 days a week.
At Eagles Landing Medical Centre, Dr. Sen, Dr.Nakamura, Dr. Bartfeld, and Dr. Schwartz are part of this Circle of Care network of physicians. We are all part of the Bathurst North Family Health Organization.
Reasoning Behind the Circle of Care Medical Model
The goal of the Circle of Care medical model is that a network of physicians works together to provide you with the highest quality of care every time you need a consultation.
When you visit a physician outside the Circle of Care network, for example, a walk-in clinic, your family doctor does not get updated on your health. This is a reoccurring problem that is inefficient and does not keep your family doctor up to date with your latest medical tests or medical history. The Circle of Care network is our chance to fix this persistent problem.
For example, if you were to see a doctor outside of our network of care, normally the x-rays, medical test results, and medical history updates are not sent to your family doctor.
This leaves a gap in your medical history. When you visit your family doctor, he/she is uninformed and not able to recommend or assist you in a timely and effective manner since they do not have the current test results or recent medical history.
Our network of doctors in the Bathurst North Family Health Organization has banded together in an effort to bring you the highest quality of care when you need it most. Once you have consulted with one of our many physicians, the consultation note and any tests ordered are immediately sent to your family doctor.
* Included in this Circle of Care are also the Vaughan Urgent Care Clinic and any emergency department in the province including Mackenzie Health Richmond Hill and Cortellucci Vaughan Hospital.
Common Questions of the Circle of Care Medical Model
How To Book Circle Of Care Family Doctors After Hours Family Doctor Appointments
An after hours appointment are available in the evenings from 5 pm to 8 pm and on weekends from 9 am to 12 pm. If your primary doctor might not be available for an appointment, you can simply book online through the green squares at the bottom of the website to book after hour appointments at your convenience.
In addition to the On Call Doctors, the Vaughan Urgent Care can be accessed Monday to Friday 4-10 pm and Saturday/Sunday from 10 am-4pm. All Emergency departments can be visited at any time. We will be updated with your medical records with either of these choices.
Please note, it is not recommended that you go to a walk-in clinic outside the Circle of Care network, call a virtual clinic such as Shoppers, Telus, or Greenshield, or call a house call doctor. These are general practitioners (GP) or family doctors that are not in your Circle of Care network. They are considered outside use and should be avoided.
Furthermore, using services outside the Circle of Care network results in the Ministry of Health in Ontario taking away valuable funding that was allocated to the clinic. These funds are used to help care for you the best we can.
As a result of these penalties, any outside use for your medical care may result in your termination from the practice.
At Eagles Landing Medical Centre, our family doctors help you take control of your health. In addition to offering treatment and prescriptions for your health issues, they can also provide guidance on things you can do to improve your health, when to go to the emergency room, or when to see a specialist in the first place.
